چکیده

C ongenitally corrected transposition of the great arteries (ccTGA) is a rare cardiac malformation characterized by the combination of discordant atrioventricular and ventriculoarterial connections. The incidence has been reported to be around 1/33 000 live births, accounting for 0.05% of congenital heart malformations. Although a familial recurrence of heart defects in subjects with ccTGA has been reported, the etiology of this malformation is not currently known. The morphological right atrium is connected to the morphological left ventricle across the mitral valve, with the left ventricle then connected to the pulmonary trunk; the morphological left atrium is connected to the morphological right ventricle across the tricuspid valve, with the morphological right ventricle connected to the aorta. Because of this “double discordance,” the systemic venous return is pumped to the lungs, while the pulmonary venous return is directed to the body. In the context of usual atrial arrangement (situs solitus), the morphological left ventricle is thus positioned to the right and the aorta, arising from the morphological right ventricle, is more commonly left-sided. In mirror-image atrial arrangement (situs inversus), the morphological left ventricle is left-sided, and the aorta is usually positioned to the right. Most patients have at least 1 associated cardiac anomaly, mainly ventricular septal defect (VSD), subvalvular and/or valvular pulmonary stenosis, or abnormalities of the systemic (tricuspid) atrioventricular (AV) valve with coexisting accessory pathways. Because of the unusual position of the atrioventricular node and course of the atrioventricular conduction bundle, cardiac conduction disorders are frequent and markedly alter the natural history and management of the malformation. This article provides a comprehensive, up-to-date description of the characteristics of the cardiac conduction system in patients with ccTGA, their clinical sequelae, and their contemporary management.

[Cardiac resynchronization in a patient with congenitally corrected transposition of the great arteries].

Congenitally corrected transposition of the great arteries is a rare heart defect that can be associated with systemic ventricular dysfunction and conduction disturbances. The use of cardiac resynchronization therapy in patients with congenital heart disease is not fully established. The authors report a case of cardiac resynchronization therapy in a 31-year-old woman with a history of congenit...

Successful pregnancies after transvenous cardiac resynchronization therapy in a woman with congenitally corrected transposition of the great arteries

Congenitally corrected transposition of the great arteries is a rare heart defect that can be associated with systemic ventricular dysfunction and conduction disturbances. The use of cardiac resynchronization therapy in patients with congenital heart disease is not fully established, and achievement of successful pregnancies after implantation of transvenous, biventricular system has never been...

Incessant bundle branch reentrant ventricular tachycardia in a patient with corrected transposition of the great arteries

Introduction Bundle branch reentrant tachycardia (BBRT) is an uncommon but clinically relevant macroreentrant ventricular tachycardia (VT) involving an impaired His–Purkinje system and the ventricle as a reentrant circuit. BBRT is commonly associated with structural heart diseases, including cardiomyopathy, old myocardial infarction, valvular disease, and myotonic dystrophy. Patients with conge...

Congenitally corrected transposition of great arteries in 76-year-old woman.

A 76-year-old woman with no cardiac history was admitted to our hospital for knee joint surgery. The ECG showed negative T wave in leads I, aVL, V3-V6. Transthoracic echocardiography showed diffuse mild hypokinesis and prominent trabeculations of the left side ventricle (Picture 1). Mild aortic regurgitation and mild left atrioventricular regurgitation were also observed. Contrast-enhanced thre...

Ventricular assist device outflow graft in congenitally corrected transposition of great arteries - a surgical challenge

Congenitally corrected transposition of the great arteries is a complex congenital cardiac anomaly with a wide spectrum of morphologic features and clinical profiles. Most patients are diagnosed late in their life, undergoes surgical repairs, eventually leading to systemic ventricular failure needing heart transplant or mechanical circulatory assistance. As, aorta is located anterior to and lef...

Successful catheter ablation of ventricular tachycardia in a patient with congenitally corrected transposition of great arteries after double switch operation

Ventricular tachycardia (VT) may cause sudden death late after repair of congenital heart disease. Radiofrequency catheter ablation (CA) of VT can be effective but may be hampered by hypertrophied myocardium or prosthetic material. A 33-year-old man with congenitally corrected transposition of the great arteries (ccTGA) had undergone a double switch operation (DSO) with the combined Mustard and...
